One of the contributing factors is the jump holes do not function correctly. Both getting to LP and coming back, the physical location of your character and the saved location of you character are not the same. The save location is correct but the system you are in is not correct. To get both to be in agreement you have to jump to LP or jump from LP as the case maybe.
